Depression-Cardiovascular Disease: Does the Arrow Point Both Ways?

COURSE OVERVIEW:
Psychosocial factors have long been reported to be independently associated with coronary heart disease. Depression is a relatively strong predictor of cardiovascular illness and may predict initial disease onset. Depression also often complicates cardiovascular events and has been found to predict mortality following motivational interviewing. Anxiety may be another factor that contributes to increased risk of cardiovascular events; however, data related to anxiety are less clear than for depression. Certainly, anxiety may predispose people to the onset of depression. Lack of social support is associated with increased cardiovascular risk and acute elevation of risk has been found in association with anger and hostile attitude. The purpose of this presentation is to provide a summary of the evidence related to depression as both a risk factor for, and a consequence of, coronary artery disease. In addition, the nursing role in terms of assessment of risk for and treatment of depression in cardiac patients will be discussed. Finally, a proposed blend of the theoretical models of patient self-management and motivational interviewing will be suggested as an ideal underpinning for nursing practice with cardiac patients who may need intervention related to emotional or behavioral issues.
