MEETING OVERVIEW:
Join us for a half-day program to examine: the prevention of stroke through the assessment of inflammatory biomarkers; the link between patients with CVD and chronic kidney disease and the role of blood pressure; and behavior change theories as related to exercise and the challenges of medication adherence, with a focus on the older cardiovascular patient. Learn how to use the information presented in your practice and improve outcomes for cardiovascular disease prevention.

Upon completion of this program, participants will be able to:

  1. Categorize a patient's risk status for stroke as high risk, intermediate risk, or low risk
  2. Describe the role of inflammatory factors in atherosclerosis and ischemic stroke
  3. Given a case example, identify two ways to lower stroke risk
Cardiovascular Disease and Chronic Kidney Disease: A Powerful Connection
  1. Examine the link between patients with CKD and CVD
  2. Identify patients at risk for CKD, review screening for CKD, and assessment of eGFR
  3. Explain how blood pressure affects the progression of CKD and CVD
  4. Examine the role of home BP monitoring in slowing progression of CKD and CVD
Behavior Change Theories/Strategies to Promote Exercise Behavior Change and Medication Adherence in the Older Cardiovascular Patient
  1. Describe various behavior change theories and their application to exercise behavior change and medication adherence in the older cardiovascular patient
  2. Identify the challenges of patient medication adherence
  3. Integrate the use of PCNA's clinical forms/tools into practice in order to better serve the older cardiovascular patient in effectively changing exercise behavior and medication adherence strategies
  4. Using a complex case study presentation, implement assessment and management strategies for older cardiovascular patients with exercise behavior change needs and medication noncompliance

The audience for this course consists of nurses and nurse practitioners specializing in cardiovascular disease prevention and management.
